Bristol Elder Services, Inc., located at 1 Father DeValles Blvd., Fall River, MA, is a private, non-profit agency that provides community-based support services to seniors. The agency is hiring a qualified candidate to recruit, hire, train, and supervise staff to transport noon time meals Monday through Friday to homebound elders and to serve meals at congregate sites. Responsibilities include traveling to meal sites in 15 communities and accompanying drivers on delivery routes, a high school diploma, a minimum of 2 years supervisory experience, and reliable transportation. Supervision of paraprofessional or volunteer workforce is preferred. This full time position offers a competitive benefits package. Portuguese Speaking Geriatric Support Services Coordinator e20120317

The agency is seeking a Geriatric Support Services Coordinator to conduct in-home psycho-social assessments and develop and implement community care plans for Portuguese speaking elders participating in a coordinated care health insurance program. Work with interdisciplinary team, including primary care physician and RN. BA/BS, and at least six months elder care experience required. Social work license preferred.
